Primary Procedures
- Upper Blepharoplasty: This procedure focuses on removing the excess skin and sometimes fat tissue from the upper eyelid, which causes a sagging, “hooded” appearance. The incision is hidden within the natural crease line of the eyelid, making the scar virtually invisible.
- Lower Blepharoplasty: Designed to address under-eye bags and puffiness. In this procedure, excess fat tissue is removed or repositioned, and sagging skin is tightened if necessary.
- Combined Blepharoplasty: This is the simultaneous application of both procedures for patients who have significant signs of aging on both the upper and lower eyelids. This provides a more holistic and balanced rejuvenation.
Patient Roadmap: A Step-by-Step Chronology
The patient’s surgical process is a well-planned journey, from the first contact to seeing the final results. A clear understanding of each stage of this journey eliminates uncertainty and increases patient comfort.
Stage 1: Pre-Operative Protocol
- Consultation: The process begins with a detailed consultation between the patient and the surgeon. This meeting can be held online or in person. At this stage, the surgeon evaluates the patient’s eye and facial anatomy, listens to their aesthetic goals, and reviews their medical history.
- Preparation: A clear preparation list is provided for the patient to follow in the weeks leading up to the operation. This list typically includes stopping blood-thinning medications (aspirin, some anti-inflammatories) and herbal supplements. Additionally, stopping smoking and alcohol consumption is strongly recommended to support optimal healing.
Stage 2: Surgical Day
- Anesthesia: The appropriate anesthesia method is chosen for the patient’s comfort and safety. Blepharoplasty is generally performed under local anesthesia and sedation (a light state of sleep). This ensures the patient feels no pain during the operation but means they are not completely unconscious. In some cases or for combined procedures, general anesthesia may also be preferred.
- Duration: The duration of the operation varies depending on the scope of the procedure. It can take about 1 hour if only the upper or lower lids are being addressed, or 1 to 3 hours for a combined procedure.
- First Moments After Operation: When the surgery is complete, the patient may have protective tapes on their eyes. A feeling of mild tightness or itching in the eyelids is normal. The patient is kept under observation by attentive medical staff during these first few hours.
Stage 3: Recovery Process (Detailed Timeline)
The recovery process is a gradual one that requires patience and proper care. Understanding this process helps patients manage their expectations correctly.
- First 24-48 Hours: Rest is essential during this period. Keeping the head elevated with a few pillows helps reduce swelling. Applying cold compresses at specified intervals is the most effective method for controlling bruising and edema. Prescribed eye drops or ointments should be used regularly. It is normal to experience temporary blurred vision and sensitivity to light during this time.
- Week 1: Swelling and bruising may be at their peak at the beginning of this week but will gradually start to decrease. If non-dissolvable stitches were used, they are generally removed between the 5th and 7th days. Patients can return to light activities by the end of this week.
- Weeks 2-4: The majority of the swelling and bruising disappears during this period. Most patients can return to their work and social routines. However, heavy exercise, bending, and strenuous activities that could increase blood pressure should be avoided for a few more weeks.
- Months 1-6: The final results become apparent during this period. Any remaining mild swelling completely subsides, and the incision scars fade to become nearly invisible. The definitive and settled result of the procedure is generally fully visible after 6 to 8 months.
A Transparent Discussion on Risks and Complications
Like any surgical procedure, blepharoplasty involves potential risks and side effects. Being transparent about this is fundamental to building patient trust and obtaining informed consent.
Common and Temporary Side Effects
There are some conditions that the vast majority of patients experience after the operation, which are a normal part of the healing process. These include swelling, bruising, dryness or watering of the eyes, a feeling of numbness or tightness in the eyelids, and temporary blurred or double vision. These symptoms generally resolve on their own within the first few weeks.
Potential Complications
Although they occur more rarely, there are potential complications that patients should be aware of. These include infection, bleeding, adverse reactions to anesthesia, visible scarring, difficulty in fully closing the eyes, and the pulling down or turning outward of the lower eyelid (ectropion). The likelihood of these risks can be significantly reduced when the procedure is performed by a highly experienced, specialized surgeon (preferably oculoplastic) in a properly equipped hospital.
Asian Blepharoplasty (“Double Eyelid” Surgery)
When it comes to ethnic blepharoplasty, the most commonly performed procedure is the “double eyelid” surgery for patients of Asian descent. In approximately 50% of individuals of East Asian descent, the fold known as the supratarsal crease on the upper eyelid is not naturally present (“monolid” or single eyelid). Furthermore, Asian eyelid anatomy has structural differences, such as the distribution of orbital fat and the insertion point of the orbital septum. The goal of this surgery is to create a natural-looking eyelid crease appropriate for these anatomical features, making the eyes appear larger, more defined, and more expressive.
Asian Blepharoplasty Techniques
- Full Incision Method: This is the most suitable method for patients who have excess skin and fat tissue on the upper eyelid. A full incision is made along the planned crease line. This offers the most permanent and predictable results.
- Partial Incision Method: Ideal for patients who do not have excess skin but do have excess fat. As it involves smaller incisions, it leaves less scarring and offers a balance between permanence and minimal invasiveness.
- Non-Incision Method (Suture Method): Suitable for patients who do not have excess skin or fat and only want to create a crease. No incision is made on the skin; instead, special sutures are passed under the skin to create a connection between the skin and the underlying muscle, thereby forming the fold. It has the fastest recovery time, but its permanence may be less than other methods.
Customization
In this procedure, the height and shape of the crease are fully customized according to the patient’s preferences and facial structure. For example, patients of North-East Asian descent (China, Korea) generally prefer a lower crease (3-6 mm), while patients of South-East Asian descent (Thailand, Vietnam) may find a higher crease (7-10 mm) more aesthetically appealing.
Upper Eyelid Surgery
It is easy to tell if there is anything wrong with the upper eyelid by gently pressing your hand on your eye. The pressure will reveal any fat in the eyelid, and the requirement of surgery. Cosmetic upper eyelid surgery usually includes correcting the eyebrows, as these two structures complement each other.
Upper eyelid surgery can be performed with local anesthesia, and should be conducted under hospital conditions. The duration of the actual procedure is quite short, and it is commonly combined with lower eyelid surgery. The cost and price of the operation will be determined by the shape of the eye.

Almond-Shaped Eye Surgery
Almond-shaped eyes are something I strongly recommend to every eyelid surgery patient. An almond-shaped eye is slightly slanted. We create this look by pulling the lower and upper eyelids slightly backwards to create slanting.
People are known to be very sensitive to the aesthetic appearance of the eyes. They try and solve their problems using ointments, medication and facial masks, only to create temporary solutions. But it is obvious that none of these methods will yield the long-lasting results of cosmetic surgery.
Cosmetic eye surgeries need to be planned by surgeons specializing on aesthetic and plastic surgery. Since this operation is a type of cosmetic surgery, it would be inappropriate for it to be performed by physicians of other branches. Treatment outcomes will be significantly more satisfactory if the surgery is performed by a physician that is an expert in eye surgery. The ability of the surgeon will directly reflect on the results of the operation.
The patient must limit showering for a while immediately after the surgery. The patient can be discharged from the hospital on the same day. Your physician might prescribe you some medications for the recovery process. We advise our patients to come to routine follow-ups for the first month after the operation to ensure proper recovery. Revision Blepharoplasty
In some cases, a revision (corrective) surgery may be necessary years after the first operation or if the results of the initial surgery are unsatisfactory. Common reasons for revision blepharoplasty include: aesthetic dissatisfaction (asymmetry, insufficient or excessive tissue removal), functional problems (inability to close the eyelids completely), or simply the formation of new sagging due to the natural effects of the aging process. Revision surgery is a more complex and delicate procedure than the initial operation. Turkey is also considered a leading destination for revision blepharoplasty due to its high concentration of experienced surgeons specialized in such complex corrective operations.
Non-Surgical Eyelid Treatment Alternatives
For patients who do not want surgical intervention or who have mild issues, effective non-surgical options are also available to rejuvenate the eye area. These methods can also be used to improve or maintain surgical results.
Who Are They Suitable For?
Non-surgical methods are generally ideal for patients with mild to moderate skin laxity, those who do not yet feel ready for surgery, or those who have medical contraindications to surgery. Furthermore, they can be applied to support the results obtained after surgery and increase their longevity.
Primary Methods
- Plasma Energy (Plexr): This technology creates a small electrical arc between the device’s tip and the skin surface, ionizing the gases in the air (especially nitrogen) and releasing plasma energy. This energy sublimates (turns directly from solid to gas) the outermost layer of the skin, triggering collagen production in the underlying tissue and providing skin tightening. It is particularly effective for mild eyelid drooping.
- Radiofrequency (Thermage): Radiofrequency energy heats the deep layers of the skin in a controlled manner, tightening existing collagen fibers and stimulating new collagen production. Over time, this provides a visible tightening and lifting of the skin.
- Injectable Fillers: Hyaluronic acid-based fillers are used, especially for volume loss and hollowness under the eyes (tear trough). These fillers fill the area, providing a smoother, brighter, and more rested appearance.
- Botox (Botulinum Toxin): It temporarily relaxes the dynamic muscles that form crow’s feet lines at the corners of the eyes, smoothing these wrinkles. Additionally, it can create a “chemical brow lift” effect by slightly lifting the outer corner of the eyebrows, providing a more open expression in the eyes.
